Rav Huna bar Menuch [in the name of Rav Acha, the son of Rav Ika, in Gittin (24a, 64b), Megillah (25b), Bekhorot (28b, 45a), Bava Kamma (58b), Bava Metzia (71b), Sanhedrin (42a). Indeed, in Bava Kamma (22a) it is in the name of Rav Ika; perhaps it should be in the name of Rav Acha bar Ika. And in Yevamot (43a), it is in the name of Rav Idi, the son of Rav Ika, which is a typographical error and should be Acha.
