Rabbi Shimshon of Chinon authored Sefer Keritot in the year 5072 [(Tzeda)]. And in Shomer HaDat, letter 52, section 2, it is written he was called Maharshak, who wrote Tosafot through his student Rabbi Elazar of Tuch, and they called it Tosafot Tuch, but the Yuchsin wrote that it was Rabbi Shimshon bar Yitzchak and he died in the year 5070, and so it concludes]:
