Rabbi Avin bar Rav Ada said in the name of Rabbi Yitzchak [Pesachim (8b), Berakhot (6a), Eruvin (8a, 11a, 39b), Bava Kamma (116b), Niddah (66b) and above, Abba bar Ada said in the name of Rabbi Yitzchak Shabbat (110a) is certainly a transcription error and should be Rabbi Avin bar Ada; and in the Rif, Rabbi Avin said in the name of Rabbi Ada in the name of Rabbi Yitzchak is also a transcription error. And in Pesachim (34a) Ravin bar Rav Acha said in the name of Rabbi Yitzchak is undoubtedly a transcription error and should be bar Rav Ada].
